Background
The Social Determinants of Health (SDoH) survey is an annual requirement of The Centers for Medicare & Medicaid Services (CMS) for all ACO REACH practices to obtain data and then address health disparities for their Medicare beneficiaries. The more patients who take the survey, the bigger the impact. With accurate documentation, practices can get the appropriate resources to best support patients.
Objective
- Collect SDoH data while ensuring a seamless experience for both practices and patients
- Deliver clean data to CMS by the established deadline
- Collect data from 100% of PY25 REACH ACOs to achieve maximum quality earn back.
Strategy
- Create communication series with three different outreach mediums (SMS, Email, Direct Mail) to increase the odds of engagement
- Remove survey submissions from remaining communications
- Send data to practices weekly so they can address identified issues and provide assistance before the end of the year
